Requirements
This outlines the appropriate requirements for obtaining an Anesthesiologist Assistant license. The Board reserves the right to make whatever additional demands on the applicant for licensure the Board deems appropriate at the time. Below is a summary of what is listed in the rules of Chapter 32 of the North Carolina Administrative Code.

1. Completed application form.
- CIRCLE the correct answer for all questions.
- Provide DETAILED explanations for affirmative answers. A claim form must be completed for EACH malpractice suit or settlement (form enclosed-photocopy as needed). Attach a photocopy of plaintiffs complaints and settlement orders for each incident.
- Sign applicant’s oath and have signature NOTARIZED. Make sure the original application form is submitted to the Board. Applications will be sent back to the applicant if the dates do not match.
- Include name change documentation, if applicable.
2. One Certified Photograph: no smaller than 2 ½” x 3 ¼”, head and shoulders shot, taken within the past sixty (60) days, of durable quality, (not a snapshot or home Polaroid) to be certified by your anesthesiologist assistant program on the Anesthesiologist Assistant Program Certification form. This certification must bear the original signature of the Dean or other Official of your program, their official title and the seal of the school must be placed over the photo and mailed to the Board’s address. THIS REQUIREMENT CANNOT BE WAIVED.
3. You must secure a report from each state, Canadian province or US territory regarding status of licensure. All licenses, active and/or inactive, must be verified. Most licensing agencies charge a fee for this service. The verifications should be sent directly to the NC Medical Board. NCMB accepts license verification through the veridoc service.
- If you have ever been licensed in Connecticut, you must send the consent for release of confidential disciplinary records form, along with the NC licensure verification form to the Connecticut Department of Public Health. If you have never been licensed in Connecticut, disregard the form.
4. Verification of certification from the National Commission for the Certification of Anesthesiologist Assistants (NCCAA). Verifications may be obtained by going to the NCCAA website (http://www.aa-nccaa.org/cgi-bin/cert.pl).
5. Three current reference forms. These forms must be sent from the reference sources directly to the North Carolina Medical Board.
- The forms must originals with original signatures of the authors, addressed to the NC Medical Board (not “To Whom it may concern”).
- At least one form must be from an anesthesiologist with whom you have worked or trained regarding your competence to practice as an AA.
- Recommendations must not be from a relative.
6. Applicants must submit two completed fingerprint cards along with their application form, application fee and authority for release of information form for the purpose of conducting a criminal background check. Expect a minimum of 8 weeks for the report to be received. Since rejections are common, the SBI has suggested that applicants use lotion or witch hazel on their hands before being fingerprinted. Fingerprint cards are submitted for processing twice a week. The SBI has suggested that using live scan when available may be a more reliable choice. When possible have different officials complete each card.
7. Data Bank Query (applies to all pathways). NCMB staff will request a current report from the National Practitioner Data Bank (NPDB) AND the Healthcare Integrity Practitioner Data Bank (HIPDB)
8. Continuing Medical Education. Documentation must be provided to show that the Anesthesiologist Assistant is in compliance with all continuing education requirements and recertification requirements of the National Commission for the Certification of Anesthesiologist Assistants or its successor organization.
Registration. North Carolina Law requires licensed anesthesiologist assistants to register with the Board within 30 days of their birth date, every year, no matter when the license is issued. A registration fee is required.
Process
Our process involves the collection of credentials from you the applicant, and from other sources as well. Once all application materials have been collected, our staff must review them. After reviews have been done, it may be necessary for the staff to request additional information, and/or to return items that need corrections or clarifications. It is our goal to review materials as quickly as possible, but we must be thorough. You should not expect the entire process to take less than four months. You must have successfully completed the NCCAA examination to qualify for licensure. Please adhere to the following guidelines:
1. Credentials received prior to receiving your application will be held in our office for one year. Items more than a year old will be considered expired.
2. Processing time may take longer during the months of March through August, due to the increased number of applications received at the North Carolina Medical Board.
3. Without an anesthesiologist assistant license and a primary supervising anesthesiologist, you cannot practice as an anesthesiologist assistant in North Carolina. Do not make practice commitments prematurely.
4. If you have been named in any malpractice suits, or have answered “yes” to any of the questions on our application, please include detailed information and anticipate the possibility that additional information will be requested and a personal interview with the Board may be required.
5. State law will only allow us to speak to the applicant about the application. Relatives, staff members, future employers, placement services and insurance companies cannot be informed about the status of your application.
6. After your application has been received and reviewed you will be notified. You will be provided with a status update regarding deficiencies.

Fees
A fee of $190.00 U.S. dollars is to be paid at the time the application is submitted. ($150.00 for the license application fee, $38.00 for the criminal background check fee, and $2.50 for the NPDB report fee). Personal checks made payable to the NC Medical Board are acceptable. Checks returned for insufficient funds will require an additional $20.00 fee. Returned checks must be replaced by a certified check or money order. Fees received are not refundable. Applications will not be processed until an application fee has been received.

IMPORTANT: NCMB now provides two different pathways for individuals who wish to create or manage a NC-based Professional Corporation or Professional Limited Liability Company (PLLC).
If you are a licensee (physician or PA) of the North Carolina Medical Board, please establish your entity of choice by clicking on the Licensee Portal button at right.

If your medical practice is registered as a professional corporation or professional limited liability company in another state, you must meet certain criteria set forth in North Carolina state laws and rules and register with the N.C. Medical Board and the N.C. Secretary of State’s office in order to practice in North Carolina.
It is important that you familiarize yourself with the NC Laws (Medical Practice Act and Professional Corporation Act) and Rules (Corporations) prior to applying to the Medical Board for registration and certification as a Foreign Professional Corporation or Professional LLC for the practice of medicine in North Carolina. Once documents are reviewed by NCMB and it has been determined that all statutory requirements are met, (see the Process Tab) NCMB will email a scanned copy of (1) the Application for Certificate of Authority, (2) NCMB issued Certificate (NCMB PC-02 or NCMB PLLC-02) and (3) the Certificate of Existence/Good Standing from the state of origination for filing with the NC Secretary of State. All other documents generated during the approval process will also be emailed to you as scanned documents for you to retain for your records.
Creation Filing with the NC Secretary of State (“SOS”) requires the Articles with the NCMB dated/stamped seal, the Board issued Certificate, the Certificate of Good Standing from the state of origination and the required SOS fee. Filing with the SOS must be completed within four months of the NCMB Certificate issued date. NCMB verifies with the NC SOS that your business has submitted a Creation Filing within four months of certification.
To Complete Creation Filing with the NC Secretary of State, you have two options:
1. Print out the hardcopy of the scanned Corporation Documents for NC Secretary of State. These three documents are the stamped/approved (1) Application for Certificate of Authority, (2) NCMB issued Certificate (NCMB PC-02 or NCMB PLLC-02) and (3) the Certificate of Existence/Good Standing from the state of origination for filing with the NC Secretary of State. These three documents, along with the required fee will then need be mailed to the NC Secretary of State.
OR
2. File the attached stamped/approved Corporation Documents for NC Secretary of State with the NC Secretary of State using the NC SOS website: Online Submission of Documents.
Unfiled Certificates expire after four months. If certification expires, reapplication will be required before filing with the SOS.
Process
All application packets must be initiated and paid for online before submitting documents to the Board for approval. Upon receipt, application packets are reviewed to ensure that statutory requirements are being met. The following are examples of what the Board looks for during the application review.
Corporate/LLC Name: The name shall not include any adjectives or other words not in accordance with ethical customs of the medical profession. It may not be identical or so similar in name to an existing registered business entity as to be misleading.
No d.b.a. A Professional Corporation/PLLC may not use any name other than the corporate name registered with the NCMB and the Secretary of State. NCMB rules prohibit using a “d.b.a.”(doing business as) name.
Name Ending: Corporations registering with NCMB are to use Professional Corporation, Professional Association, P.C., P.A., PC or PA as their corporate ending. Do not use the following endings: corporation, company, limited, incorporated, corp., co., ltd. or inc
.
Name Ending: Professional Limited Liability Companies The name shall contain the word “professional” or the abbreviation “P.L.L.C” or “PLLC.”
Foreign Shareholder/Member Requirements: Shareholders and members must be in good standing with relevant licensing boards. “In good standing” means has not been disciplined by a licensing Board and is not currently subject to disciplinary proceedings.
(1) At least one shareholder must be currently licensed and in good standing with this Board
(2) At least one director and one officer must be currently licensed and in good standing with this Board
(3) No person other than a licensee of the Board shall exercise any authority or influence over the practice of medicine in North Carolina.
(4) All shareholders/members not licensed in North Carolina must hold a license in good standing, in their state of practice.
The professional services to be rendered:
• Physicians qualify to apply for a Certificate of Registration as a professional corporation with the purpose of practicing medicine;
• Together a physician and a physician assistant qualify to apply for a Certificate of Registration as a professional corporation with the purpose of practicing medicine;
• If the business entity is registering for additional professional services with other NC Boards, the purpose is reviewed to ensure respective stockholders/members are licensed, certified or otherwise approved to provide the services outlined in the purpose. Requirements
Beginning July 1, 2006, all perfusionists in North Carolina must obtain licensure in order to practice perfusion. There are three pathways by which a person may obtain his or her perfusion license.
Licensed Perfusionist Pathway: This pathway is for applicants who have graduated from an approved educational program, have a current certification as a clinical perfusionist, and who do not wish, or may not be qualified, to obtain a license by one of the other pathways.
Reciprocity Pathway: This pathway is available for applicants who are licensed, in good standing, as a perfusionist in another state or United States Territory, and who can satisfy one of the following two criteria:
1) The state or territory in which he or she is licensed has laws regulating perfusion that are substantially equivalent to the laws of North Carolina*; or
2) The applicant has current certification as a clinical perfusionist. * In the event the applicant seeks reciprocity by electing this criteria, the Perfusionist Advisory Committee of the North Carolina Medical Board (PAC) will obtain the laws of the other state and make a determination as to whether such laws are substantially equivalent to those of North Carolina.
Provisional Licensed Perfusionist Pathway: This pathway is for applicants who do not have certification as a clinical perfusionist but do satisfy the educational requirements for perfusion licensure, and who do not qualify for licensure under the Reciprocity Pathway. A provisional license applicant must be supervised by a licensed perfusionist, and the provisional license may not exceed a period of twelve (12) months. The provisional license applicant must complete a Designation of Supervising Perfusionist Form.
Converting A Provisional License to a Full License
A provisional licensed perfusionist who receives certification from the American Board of Cardiovascular Perfusion (ABCP) may request that his or her provisional license be converted to a full license. The provisional licensee must submit a written request for the conversion and proof of certification from the ABCP, and pay the conversion fee. The Perfusionist Advisory Committee may request additional information or conduct an interview of the applicant to determine the applicant’s qualifications.

5. Any deficiencies in your application after review will be noted on your Licensure Gateway homepage.

Fees

Section .1500 - Medical School Faculty License
21 NCAC 32 B .1501
A physician holding a Medical School Faculty License may practice only within the confines of the medical school or its affiliates. “Affiliates” means the primary medical school hospital(s) and clinic(s), as designated by the ACGME.
The Medical School Faculty License is limited to physicians who have expertise which can be used to help educate NC medical students, post-graduate residents and fellows but who do not meet the requirements for physician licensure.
A Medical School Faculty Limited License cannot be “reactivated or reinstated”. If the license is made inactive, the physician must reapply.
1. Completed application form.

- A claim form must be completed for each malpractice suit or settlement. Attach a photocopy of plaintiff’s complaint and settlement orders for each incident.
- If your name has changed at any time during your life, you will need to list your prior names and submit a copy of legal documentation (marriage certificate, divorce decree, adoption papers, etc.) supporting the name change.
2. Attach a photograph taken within the past year to the applicant’s oath. Complete the form and have the form notazrized.
3. Sign and return the Eligibility Requirements form to the NCMB.
4. Verification of Faculty Appointment form. Forward form to the medical school where you will hold your appointment. Applicant must have received a full time appointment as either a lecturer, assistant professor, associate professor or full professor at a medical school in the state of North Carolina.
5. Verification of medical education: Forward form to your medical school for completion.
(a) Transcripts - If you attended one medical school for less or more than the standard four years, or if you attended more than 1 medical school, you must submit original medical school transcripts(s) translated into English, if applicable.
6. Complete the Immigration Status Form and submit required documentation.
7. Two recommendations must be from physicians using the physician reference forms. Recommendations cannot be from a relative. These forms must be sent from the reference source directly to the NC Medical Board.
8. Submit proof on the postgraduate training verification form of completion of at least (1) one year of GME approved by one of the following:
- ACGME (Accreditation Council on Graduate Medical Education
- AOA (American Osteopathic Association);
- CFPC (College of Family Physicians of Canada)
- RCPSC (Royal College of Physicians and Surgeons of Canada
or evidence of other education, training or experience, to be determined by the Board as equivalent.

10. The NC Medical Board staff will request the following documentation on behalf of the applicant. If staff is unable to obtain this information, the applicant will be contacted and expected to have this information forwarded to the NC Medical Board.

11. Applicants must submit two completed fingerprint cards for the purpose of conducting a criminal background check. When possible, have different officials complete each card. It is recommended you have your local law enforcement office complete the fingerprinting. An application is not considered complete until results of the background check have been received. Expect a minimum of 8 weeks for the report to be received. Since rejections are common, the SBI has suggested that applicants use lotion or witch hazel on their hands before being fingerprinted. Fingerprint cards are submitted for processing twice a week. The SBI has suggested that using live scan when available may be a more reliable choice. When using live scan, prints must be printed on fingerprint cards and be submitted to the NC Medical Board. They cannot be submitted electronically. See detailed instructions for completing cards. E-mail fpc@ncmedboard.org to request a set of fingerprint cards. A set contains 2 fingerprint cards.
12. Fee of $391.00 US dollars ($350.00 application fee, $38.00 background check fee and $3.00 NPDB/HIPDB report fee) is to be paid at the time the application is submitted. Personal checks made payable to the NC Medical Board are acceptable. Checks returned for insufficient funds will require an additional $20.00 fee. Returned checks will have to be replaced by a certified check. FEES RECEIVED ARE NOT REFUNDABLE.
13. When all application materials have been received, your file will be forwarded to a staff member for quality assurance review. If the quality assurance review is complete and no additional information is needed, you file will be forwarded to a board member for review to determine whether you will be required to appear for a personal interview. Upon receipt of the Board members’ decision, your license will be issued if a personal interview is not required.
14. If a physician has been away from clinical practice 2 years or longer, they may be required to develop a reentry plan as part of the license application. It is the responsibility of the applicant to be prepared to present a program of re-training or supervision that will establish proof of competency in their chosen area or medicine. Applicants in this category will be required to appear for a personal interview.
RENEWAL - NORTH CAROLINA LAW REQUIRES LICENSED PHYSICIANS TO RENEW WITH THE BOARD WITHIN 30 DAYS OF THEIR BIRTH DATE, EVERY YEAR, NO MATTER WHEN THE LICENSE IS ISSUED. A RENEWAL FEE IS REQUIRED.

After reviews have been done, it may be necessary for the staff to request additional information and/or to return items that need corrections or clarification. You will be notified in writing regarding any deficiencies in your application once it has been reviewed. It is our goal to review materials as quickly as possible, but we must be thorough. You should not expect the entire process to take less than 4 months from the time your application is received by the NC Medical Board. Please adhere to the following guidelines:
1. Processing time will take longer during the months of March through August, due to the increased number of residents and relocating physicians. We recommend you allow four to six months of total processing time if you want a license during these months.
2. Credentials received prior to your application will be held in our office for 1 year.
3. If you have been named in any malpractice suits or have answered “yes” to any question on your application, anticipate the possibility that additional information and an interview will be requested.
4. It is recommended that you not make practice commitments prematurely.
Application credentials and fee are good for 1 year. If you are not issued a license within the year and choose to reapply for a NC license, you will be required to resubmit all application credentials and the application fee.
State law will only allow us to speak to the applicant regarding the application. Relatives, staff members, future employers or placement services can not be informed about the status of your application even if you have signed a release authorizing them to speak with the Board regarding your application.
Note: The NC Medical Board will be unable to return original or photocopies of application materials once received.

Requirements
Physicians with an active NC medical license wanting to convert to the Retired Limited Volunteer License must submit a form entitled Conversion of Full License to Retired Volunteer License
Physicians who have held a NC medical license and it has been inactive for less than six months must complete the Retired Limited Volunteer License Application (short form)
Physicians must complete the full Retired Limited Volunteer License Application if they have:
- Held a NC medical license and it has been inactive for more than six months; or
- Never held a NC medical license, but wish to obtain a retired volunteer license
No application fee is required.
All physicians holding a Retired Limited Volunteer License must maintain appropriate CME hours as required by 21 NCAC 32R .0101 - .0104
An annual registration fee will be required.

There is no application fee for a retired volunteer license. However, you will need to pay for the criminal background check which is a $38 fee.
Requirements
FCVS provides a centralized, uniform process for state medical boards to obtain a verified, primary source record of a physician’s core medical credentials. This repository of information allows a physician to establish a confidential, lifetime professional portfolio which can be forwarded, at the physician’s request, to any state medical board that has established an agreement with FCVS, hospital, health care or any other entity.
Establishing an FCVS profile takes time. Applicants are encouraged to complete their FCVS application six months before they need their FCVS profile submitted to the state medical board for licensure Learn more about FCVS.

Eligibility requirements
The following outlines what is required to obtain a full medical license. Please read carefully. Failure to submit important information may delay the application process. Learn more about licensing: Rules and statutes.
If you are a graduate from a medical or osteopathic school approved by the LCME, CACMS or COCA, you must have the following:
1. Satisfactory completed at least one year of graduate medical education approved by ACGME, CFPC, RCPSC or AOA.
2. Passed a state board licensing examination: NBME, USMLE, FLEX, COMLEX, NBOME, MCCQE or their successors. The NC Medical Board does not accept the Puerto Rico state board written examination.
Each step of the USMLE or COMLEX must be passed within three attempts. However, the Board shall waive this requirement if the applicant has been certified or recertified by an ABMS, CCFP, FRCP, FRCS or AOA approved speciality board within the past 10 years.
3. Have completed a minimum of 130 weeks of medical school. The Board shall waive this requirement if the applicant has been certified or recertified by an ABMS, CCRP, FRCP, FRCS or AOA approved speciality board within the past 10 years.
4. Submit proof of one of the following qualifications within the past 10 years:
- USMLE examination.
- MCCQE examination.
- National Osteopathic Board of Medical Examiners Examination or COMLEX.
- SPEX examination with a score of at least 75.
- COMVEX examination, the Osteopathic examination equivalent to SPEX.
- Certification or recertification of CAQ by a specialty board recognized by the American Board of Medical Specialties (ABMS), American Osteopathic Association (AOA), Certificant of the College of Family Physicians of Canada (CCFP), Fellowship of the Royal College of Physicians of Canada (FRCP) and Fellowship of the Royal College of Surgeons of Canada (FRCS).
- Completion of graduate medical education approved by ACGME, AOA, CFPC or RCPSC.
If you have not met one of the seven qualifications above, you must submit documentation of 60 hours of practice relevant CME obtained in the past three (3) years. The 60 hours shall be educational provider-initiated.
If you are a graduate of a medical school not approved by LCME, COCA or CACMS, you must have the following:
1. Current valid certification of the ECFMG or completed an approved Fifth Pathway Program.
2. Satisfactorily completed three years of graduate medical education approved by ACGME, CFPC, RCPSC or AOA.
3. Passed a state board licensing examination; NBME; USMLE; FLEX, COMLEX, NBOME, MCCQE or their successors. The NC Medical Board does not accept the Puerto Rico state board written examination.

Reentry
If a physician has been away from clinical practice two years or longer, they may be required to develop a reentry plan as part of the license application. You should review the information on the Board’s website regarding reentry.
YOU WILL NEED TO COMPLETE AND SUBMIT ADDITIONAL DOCUMENTATION AS REQUIRED IN THE ONLINE APPLICATION.
Application credentials and fee are good for 1 year. If you are not issued a license within the year and choose to reapply for a NC license, you will be required to resubmit all application credentials and fee.
RENEWAL - North Carolina law requires licensed physicians to renew with the Board within 30 days of their birth date, every year, no matter when the license is issued. A renewal fee is required.
Process

It may be necessary for the staff to request additional information, and/or return any times that may need corrections or clarifications. Our goal is to review materials as quickly as possible, but we must be thorough. You should not expect the entire process to take less than four months from the time your application is received by the NCMB. Processing does not begin until all application fees have been paid online.
Please note and adhere to the following guidelines:
1. Processing time will take longer during the months of March through August, due to the increased number of residents and relocating physicians. We recommend you allow four to six months of total processing time if you wish to obtain a license during these months.
2. You should submit your fingerprint cards and authority to release form as soon as possible so that a background check request can be made. Your fingerprint cards cannot be forwarded to the SBI until payment has been made online. Once we receive payment, you will not be able to return to your application to make corrections. Credentials received prior to the completion of your application will be held in our office for one year.
3. If you have been named in any malpractice suits or have answered “yes” to any question on your application, anticipate the possibility that additional information and an interview may be required.
4. It is recommended that you not make practice commitments prematurely.
5. Without a medical license, you cannot practice as a physician in North Carolina. Do not make commitments on loans, practice start dates, home purchases, etc., until you have been granted a license to practice medicine in North Carolina.
6. State law only allows staff to speak to the applicant about the application. Relatives, staff members, future employers or placement services can not be informed about the status of your application.
7. If you wish to exit the application at any time, please do so by clicking on the ‘Logout’ button at the bottom of the page. This will save all the date that you have entered.
8. You can check the status of your application by clicking on the ‘Check Status’ link. Once you have submitted your application, it will take up to 15 days before the information on your status is up to date. You will need to know your File ID number and your date of birth to access this information. Your File ID number will be given to you once you start the application.
Application credentials and fee are good for 1 year. If you are not issued a license within the year and choose to reapply for a NC license, you will be required to resubmit all application credentials and the application fee.
Note: State law will only allow us to speak with the applicant regarding the application. However, you may give your login information for checking the status online to whomever you choose.
The NC Medical Board will be unable to return original or photocopies of applications materials once received.
Fees
Fees must be paid at the time the online application is submitted. ($400.00 for the license application fee, $38.00 for the criminal background check fee and $2.50 for the NPDB/HIPDB report). Fees must be paid by VISA, Mastercard, American Express or Discover at the end of the online application. Fees paid are not refundable.
Requirements
Physicians holding an active North Carolina Retired Volunteer License who want to convert back to a full and unrestricted license, must complete the Board’s application form; pay the reinstatement fee and provide documentation of 60 hours of practice-relevant CME obtained in the past three years. The 60 hours shall be educational provider-initiated. An interview may be required.

After reviews have been done, it may be necessary for the staff to request additional information, and/or to return items that need corrections or clarifications. Fees
The fee for reinstating a full, unrestricted license from a retired volunteer license is $438.00 and includes a $38.00 fee for criminal history record check.
Requirements
The Limited Volunteer License is available to physicians who are not licensed in North Carolina but wish to volunteer at indigent clinics. The holder of a limited volunteer license shall practice medicine and surgery within the state of North Carolina for no more than 30 days per calendar year and may not receive compensation for services rendered at clinics specializing in the care of indigent patients.

It may be necessary for the staff to request additional information, and/or return any documents that may need corrections or clarifications. There is no application fee for the limited volunteer license. However, you will need to pay the criminal background check fee which is $38.00. A renewal fee will not be required, however, you will be required to complete the online renewal process within 30 days of your birthday every year.
Requirements
This outlines the appropriate requirements for obtaining a special purpose license/special permit. The Board reserves the right to make whatever additional demands on the applicant for license the Board deems appropriate at the time. Below is a summary of what is listed in the rules of Chapter 32 of the North Carolina Administrative Code.
The Special Purpose License is for physicians who wish to come to North Carolina for a limited time, scope and purpose, such as to demonstrate a new technique, procedure or piece of equipment, or to educate physicians or medical students in an emerging disease or public health issue.

1. Completed application form.
- Circle the correct answer for all questions and provide a detailed explanation for all affirmative answers.
- A claim form must be completed for EACH malpractice suit or settlement (photocopy as needed). Attach a photocopy of plaintiffs complaints and settlement orders for each incident.
- Attach a photograph taken within the past year to the applicant’s oath and have the form notarized.
- If your name has changed at any time during your life, you will need to list your prior names and submit a copy of legal documentation (marriage certificate, divorce decree, adoption papers, etc.) supporting the name change.
2. Verification from local facility/group to participate in a learning/teaching procedure giving details stating the name of the local facility/group where the procedure(s) will be performed, length of time of the procedure(s), type of procedure, what the procedure(s) will include and the name of the physician responsible for overseeing the procedure(s).
3. You must have verification of a full, unrestricted license mailed directly to the Board from one licensing jurisdiction. Most licensing agencies charge a fee for this service.
- If you are submitting verification of a Connecticut license as part of this application, you must send the Connecticut release form along with the NC license verification form to the Connecticut Department of Public Health.
4. Complete the Immigration Status Form.
5. Applicants must submit two completed fingerprint cards for the purpose of conducting a criminal background check. When possible, have different officials complete each card. It is recommended you have your local law enforcement office complete the fingerprinting. An application is not considered complete until results of the background check have been received. Expect a minimum of 8 weeks for the report to be received. Since rejections are common, the SBI has suggested that applicants use lotion or witch hazel on their hands before being fingerprinted. Fingerprint cards are submitted for processing twice a week. The SBI has suggested that using live scan when available may be a more reliable choice. When using live scan, prints must be printed on fingerprint cards and be submitted to the NC Medical Board. They cannot be submitted electronically. See detailed instructions for completing cards. Request a set of fingerprint cards. A set contains 2 fingerprint cards.
6. When all application materials have been received, your file will be forwarded to a staff member for quality assurance review. If the quality assurance review is complete and no additional information is needed, your file will be forwarded to a Board member for review to determine whether you will be required to appear for a personal interview. You will be notified regarding the Board member’s decision.

2. If you have been named in any malpractice suits or have answered “yes” to any question on your application, anticipate the possibility that additional information and an interview maybe requested.

State law will only allow us to speak to the applicant regarding the application. Relatives, staff members, future employers or placement services can not be informed about the status of your application even if you have signed a release authorizing them to speak with the Board regarding your application.
Note: The NC Medical Board will be unable to return original or photocopies of application materials once received.

A fee of $388.00 U.S. dollars is to be paid at the time the application is submitted. ($350.00 for the license
application fee and $38.00 for the criminal background check fee). Check should be made payable to the NC Medical Board. Checks returned for insufficient funds will require an additional $20.00 fee. Returned checks must be replaced by a certified check or money order. FEES RECEIVED ARE NOT REFUNDABLE. Applications will not be processed until application fee has been received.
A $38.00 background check fee is to be paid at the time the application is submitted. A personal check made payable to the NC Medical Board is acceptable. checks returned for insufficient funds will require and additional $20.00 fee. Returned checks will have to be replaced by a certified check. Fees received are not refundable.
Requirements
The NC General Assembly enacted Article 42, NCGS §§ 90-721 through 725, the Polysomnography Practice Act, on October 1, 2009. The statute requires the NC Medical Board to establish a registry of sleep techs. The Polysomnography Practice Act makes it unlawful, beginning January 1, 2012, for any unregistered sleep tech to practice polysomnography, represent themselves as someone credentialed to practice polysomnography or use the title “Registered Polysomnographic Technologist.”
Registrants must provide:
- Their full legal name
- Their complete address and current telephone number
- Evidence that they are currently credentialed in good standing by the Board of Registered Polysomnographic Technologists (BRPT).
The Medical Board will verify the sleep tech’s credentials with the BRPT. If the NCMB is unable to verify that a registrant is currently credentialed by the BRPT, the sleep tech may be required to provide a copy of his or her certificate.

Once documents are reviewed at the Board and it has been determined that all statutory requirements are met, (see the Process Tab) the NCMB will email a scanned copy of the Articles to you along with a NCMB issued Certificate (NCMB PC-02) for filing with the NC Secretary of State. All other documents generated during the approval process will also be emailed to you as scanned documents for you to retain for your records.
Creation Filing with the NC Secretary of State (“SOS”) requires the Articles with the NCMB dated/stamped seal, the Board issued Certificate and the required SOS fee. Filing with the SOS must be completed within four months of the NCMB Certificate issued date. NCMB verifies with the NC SOS that your business has submitted a Creation Filing within four months of certification.
To complete Creation Filing with the NC Secretary of State, you have two options:
1. Print out the hardcopy of the scanned Corporation Documents for NC Secretary of State. These two documents are the stamped/approved Articles of Organization and the Certification by the North Carolina Medical Board, with the signature of the Executive Director and seal of the Board. These two documents, along with the required fee will then need be mailed to the NC Secretary of State.
OR
2. File the attached stamped/approved Corporation Documents for NC Secretary of State with the NC Secretary of State using the NC SOS website Online Submission of Documents link.
Unfiled Certificates expire after four months. If certification expires, reapplication will be required before filing with the SOS.
Letter of Non-Objection
NC General Statute 55B prohibits any non-licensees of the NC Medical Board from forming an entity offering the professional service of the practice of medicine.
Many business entities try to file creation articles with the NC Secretary of State and the filing is rejected due to a word or words in the name of the business entity which implies the corporation/company is a medical practice.
If your business has received a rejection letter from the NC Secretary of State for this reason, you will need to request a Letter of Non-Objection from the NC Medical Board. Please be advised that requests for Letters of Non-Objection are occasionally denied by the NC Medical Board Legal Department.
To request a Letter of Non-Objection, please submit a letter on corporate letterhead that includes the following:
- State the purpose of the business in the state of NC, i.e., what will the business be doing in NC?
- State that the business will not be offering the professional service of the practice of medicine.
- State that the business will not be employing any licensees of the NC Medical Board for the purpose of offering the professional service of the practice of medicine.
The request letter should be signed by an office of the corporation.

Process
The Medical Board requires that all applicants complete the online application prior to emailing their documents to the Corporations division at corp.application@ncmedboard.org.
Upon receipt, application packets are reviewed to ensure that statutory requirements are met. The following are examples of what the Board looks for during its application review.
The original Articles of Incorporation (Professional Corporation)
Corporate Name: The name shall not include any adjectives or other words not in accordance with ethical customs of the medical profession.
Corporate Name Ending: Corporations registering with NCMB are to use Professional Corporation, Professional Association, P.C., P.A., PC or PA as their corporate ending. Do not use the following endings: corporation, company, limited, incorporated, corp., co., ltd. or inc.
Corporate Name: A professional corporation may not use any name other than the corporate name registered with the NCMB and the Secretary of State. NCMB rules prohibit using a “d.b.a.”(doing business as) name. See NC Administrative Code § 21 NCAC 32C .0102.
Incorporators: At least one incorporator must be a NCMB licensee. One of the incorporating shareholders is required to sign the Articles before submitting them to NCMB.
The specific purpose for which the corporation is being formed:
- Physicians qualify to apply for a Certificate of Registration as a professional corporation with the purpose of practicing medicine;
- Together a physician and a physician assistant qualify to apply for a Certificate of Registration as a professional corporation with the purpose of practicing medicine;
- One hundred percent physician assistant owned corporations qualify to apply for a Certificate of Registration as a professional corporation with the purpose of performing medical acts, tasks and functions. See the FAQ section for more information;
- When the corporation is to be registered with other NC Boards, the purpose is reviewed to ensure respective stockholders are licensed, certified or otherwise approved to provide the services outlined in the purpose. See the FAQ section for allowable combinations

From time to time, professional corporations have changes that must be reported to the NCMB and/or Secretary of State, such as a corporation name change or stock transfer. Instructions for reporting such changes are listed below.
Instructions for filing documents to issue or transfer stock or membership
All issuance or transfer of stock in a professional corporation requires certification of licensure from the NC Medical Board. Prior to the issuance or transfer of stock you must obtain NCMB certification by submitting the following to NCMB.
Online
• Through NCMB’s Corporation Update portal you will be able to update shareholder information and obtain certification for new qualified shareholders. Online is only available for certification of NCMB Licensees.
Or By email send to NCMB
• A letter, signed by an officer or legal representative of the corporation, stating the name of the corporation and the name of the individual licensed to practice medicine in NC who will be issued or transferred stock.
Upon execution by this Board certification (PC-05 or PLLC-05) will be issued for each newly added licensee. Certification must be permanently retained by the corporation
Name Change Request
To change the name of your professional corporation you must first email scanned copies of the following documents to corp.application@ncmedboard.org:
• A cover letter requesting a name change
• The original Articles of Amendment; The amendment must be signed by an officer of the corporation who is also licensed by NCMB (e.g. a physician or a PA).
After review, NCMB will email you a Letter of Non‐Objection authorizing the name change and the scanned copy of the original Articles of Amendment with the Board’s dated seal. Using the NC SOS website Online Submission of Documents link, you will then file the approved Articles of Amendment and Letter of Non‐Objection with the Secretary of State within four months.
Change of Registered Agent
Please notify the Board if you change the Registered Agent of your business entity by either:
• Submitting the agent change through the NCMB’s Corporation Update portal
• Emailing NCMB a copy of the Articles of Amendment for Agent Change filed with the Secretary of State.
Dissolution of a professional corporation or limited liability company
To remove your business entity from the active roll of professional corporations due to closing, acquisition or etc. you are required to complete and file Articles of Dissolution with the Secretary of State. Send a copy to the NCMB Corporations Coordinator.

Corporate Name: The name shall not include any adjectives or other words not in accordance with ethical customs of the medical profession.
Corporate Name Ending: The name shall contain the word “professional” or the abbreviation “P.L.L.C” or “PLLC.”
Corporate Name: A professional limited liability company may not use any name other than the company name registered with the NCMB and the Secretary of State. NCMB rules prohibit using a “d.b.a.”(doing business as) name.
Organizers: At least one organizer must be a NCMB licensee. One of the organizing members is required to sign the Articles before submitting them to NCMB.
The specific purpose for which the PLLC is being formed:
- Physicians qualify to apply for a Certificate of Registration as a professional limited liability company with the purpose of practicing medicine;
- Together a physician and a physician assistant qualify to apply for a Certificate of Registration as a professional limited liability company with the purpose of practicing medicine;
- One hundred percent physician assistant owned corporations qualify to apply for a Certificate of Registration as a professional limited liability company with the purpose of performing medical acts, tasks and functions.
- When the corporation is to be registered with other NC Boards, the purpose is reviewed to ensure respective members are licensed, certified or otherwise approved to provide the services outlined in the purpose.

From time to time, professional corporations have changes that must be reported to the NCMB and/or Secretary of State, such as a corporation name change or stock transfer. Instructions for reporting such changes are listed below.
Instructions for filing documents to issue or transfer stock or membership
All issuance or transfer of stock in a professional corporation requires certification of licensure from the NC Medical Board. Prior to the issuance or transfer of stock you must obtain NCMB certification by submitting the following to NCMB.
Online
• Through the NCMB’s Corporation Update portal you will be able to update shareholder information and obtain certification for new qualified shareholders. Online is only available for certification of NCMB Licensees.
Or By mail send to NCMB
• A letter, signed by an officer or legal representative of the corporation, stating the name of the corporation and the name of the individual licensed to practice medicine in NC who will be issued or transferred stock.
Upon execution by this Board certification (PC-05 or PLLC-05) will be issued for each newly added licensee. Certification must be permanently retained by the corporation
Name Change Request
To change the name of your professional corporation you must first submit to NCMB:
• Submit a cover letter requesting a name change and the original Articles of Amendment
The amendment must be signed by an officer of the corporation who is also a licensee of this Board.
• After review, the NCMB will return to you a Letter of Non‐Objection authorizing the name change and the original Articles of Amendment with the Board’s dated seal.
• File the approved Articles of Amendment and Letter of Non‐Objection with the Secretary of State within four months.
Change of Registered Agent
Please notify the Board if you change the Registered Agent of your business entity by either:
• Submitting the agent change through the NCMB’s Corporation Update portal
• Sending NCMB a copy of the Articles of Amendment for Agent Change filed with the Secretary of State.
Dissolution of a professional corporation or limited liability company
To remove your business entity from the active roll of professional corporations due to closing, acquisition or etc. you are required to complete and file Articles of Dissolution with the Secretary of State. Send a copy to the NCMB Corporations Coordinator.
