Before moving forward with filing your complaint, please read the information and understand that:
- If you wish to file a complaint against multiple licensees, you must file a separate complaint for each licensee.
- If you have new or additional information that needs to be added to your initial complaint, you will need to email the Complaints Department with that information and be sure to reference your assigned case ID#. This email does NOT accept initial complaint submissions and any initial complaint submitted will not be accepted. You must file your complaint using the online complaint submission form.
- The Board will not investigate duplicate complaints made by the same complainant against the same licensee. Filing a duplicate complaint will not expedite the Board’s investigation and may actually slow the process significantly.
- The complaint review process can take up to six (6) months or more.
Notes on Medical Records
- Failure to supply the correct provider names, facility names, addresses and dates of services may result in the Board’s inability to investigate the allegations in your complaint.
- Facility includes but is not limited to, hospitals, diagnostic testing labs, a primary care physician, and/or any other specialist that were involved in the care of the patient that is subject to the allegations in the complaint.
- If your complaint will reference multiple health care providers, facilities, clinics, or hospitals where you (or the patient) received medical care related to the complaint, you MUST provide the names, addresses and date(s) of services of EACH facility and/or doctor that treated you.
