NCMB is continuing to receive calls and inquiries from licensees who have had difficulty registering for or using the state's new electronic death registration system.
If you have not yet registered with the NC Database Application for Vital Events (NC DAVE) to submit death certificates online, you may do so by clicking the link below. NOTE: you will need to enter a security code (provided on the web page) to access the registration page. On the registration page, you will be prompted to look up your practice or facility. If your practice is not listed, you may need to contact NC Vital Records or do some detective work within your organization to get your practice or facility set up. Vital Records has provided this instructional sheet to help with the process.
